Documentation of your TB skin test:

• Use the form provided to you at your clinical compliance session OR
• Use the form given to you at Cabrillo College Student Health Services
• Make sure that ALL information on the form is complete.

Documentation of your Titers:

• Upload the lab results showing the name of the titer, the values of each titer and the results (positive, negative, equivocal)
• Make sure your name and date of birth on on the printout

Documentation of your Shots:

• Upload a picture of your yellow immunization card (front and back with your name and date of birth completed on the card) OR
• Upload a printout from an electronic medical records. Make sure your name and date of birth are on the form and that the form has the letterhead or some identification of the medical facility

Documentation of your Flu Shot:

• Upload a picture of your yellow immunization card (front and back with your name and date of birth completed on the card) OR
• Upload a printout from an electronic medical records. Make sure your name and date of birth are on the form and that the form has the letterhead or some identification of the medical facility OR
• Upload the printout from the pharmacy or drugstore where you received the immunization. Make sure the document has your name and date of birth on it

Documentation of your Physical Exam:

• Upload a picture of your COMPLETED physical exam form given to you at your clinical compliance session
