If you have completed 2 full shot series of MMR, Varicella or Hepatitis B and your titer remains negative, non-immune or equivocal you are considered a Non-Responder to that immunization.

Please contact the Clinical Compliance Coordinator for a Non-Responder form. This form will need to be filled out by your and your provider.
